Description of SVRT Program Outcome Form
This is a table which tracks an individual's needs for transportation, personal care attendant services, case management and other services as specified. For transportation services, it tracks the location of each trip (within the city or within the county) and the number of miles of each one-way trip. It records the number of trips taken per week and per day, and the cost of each trip. It also records the number of trips that the individual needs to take per week and per day and the projected cost of each.
